The Role: Reporting to the Practice Manager, the Registered Nurse is responsible for providing exceptional hands on care to patients from within their community, conducting health assessments to assist with the early detection of chronic disease, preparing management plans with patients on behalf of GPs, providing coaching for patients on health change principals and providing suitable action plans to reach specified goals and management of chronic disease.
